Documents Required

You may first download and print the the Application Form. Attach self-attested photocopies of following documents to the Application Form:


  • Indicate which service is required by ticking on one or more of the services enumerated on the first page.
  • Passport in original with self-attested photocopy of its first two and last two pages.
  • Enclose a letter explaining the requirement of specific service by the Embassy.
  • Completely fill the form from column N°1 to 10 of Section-I.
  • Completely fill Section-II and sign in the space indicated.
  • Please do not fill answers like NA, N/A, Nil or dash, kindly explain if you do not fill any column.
  • Enclose a copy of residence permit / DNI / cédula / precaria / visa (in case of DNI / Cedula Cards copies of front back sides should be enclosed)
  • 2 Photos size 3.5cm x 3.5 cm. One pasted in the designated space on the first page.
  • Applicable fee.
